Physician Referral Form

Premier Physical Therapy works with physicians of many different specialties, throughout the NYC Tri-State (NY­/CT/­NJ) area. While the majority of our patients are referred to us by a physician, occasionally we receive inquiries from direct-access patients, past patients, and family or friends of current patients, asking us for a physician recommendation.  If you or someone you know does not have a physician and would like a recommendation, please fill out the form below and Premier's Physician Liaison will contact you.
